The main objective of the service is to help people to maintain their independence in their own homes after a stay in hospital or after a period of illness. With this support, it is hoped that people will regain their confidence and prevent readmission to hospital. Anyone over the age of 65 will be considered for the service, even if they live with someone and have other services, such as, home care.
After the initial assessment, the patient will be offered information and advice as appropriate, for instance, how to get home care, how to apply for a blue badge for parking, or how to get a Careline system. Also, they may want be referred to another agency, such as, welfare benefits with view to applying for Attendance Allowance.
Many people also require a volunteer for extra support and assistance for up to six weeks. Apart from providing companionship, a volunteer can collect prescriptions, check that there is enough food and heating in the house, escort people to their out-patients appointments, accompany them to the supermarket, etc.
